Osteochondroma is most common benign tumor of primary bone tumor, occurs in metaphysis of long bone. It has translucent cartilage cap, and appear to protrude of bone. It rarely occurs in vertebra column. We report one case of osteochondroma in the 12 thoracic vertebra. Muscle of patient right leg weaker than left, and sensory and motor power were also diminished by physical examination. We can¡¯t find any specifics regarding this patient after blood test. Radiologic exam showed a bony lesion in the 12 thoracic vetebra. The differential diagnosis includes simple bony lesion, osteochonroma, etc. We performed excision and posterior decompression and posterior fusion. The mass, bony trabecula was covered by cartilage cap. On histological examination, enchondral ossificaion present in surface cartilage cap and malignant formation was not seen.. Based on histological examination, MRI, clinical examination, we made a diagnosis that it was primary osteochondroma in thoracic vertebra.
